Accident Investigation leading to Root Causes
Safe-Net International provides the skills and knowledge necessary to conduct an accident/incident investigation identifying root causes.
The training objective is to provide attendees the skills to prevent occurrence/recurrence of an accident or injury through an accurate identification of causes, both direct and indirect.
Typically what we have found within our client base is that incomplete Investigation reports are being prepared failing to discover the root cause(s) of accidents/incidents. Remedial activities are not being accurately identified and monitored, incidents reoccur and eventually Incidents are not being reported at all resulting in a continuation of loss.
Safe-Net International’s approach to this dilemma ensures a strong determination of cause and the effects of loss at the onset, giving staff the ability to conduct effective investigations from initial response to a thorough gathering of evidence to re-constructive interviews.
