A partial example of assessing an individual’s responsibility for safety within an organization’s Safety Management System:
Please respond to the following questions to the best of your knowledge:
1. Are loss control responsibilities and objectives (Safety objectives particularly) clearly defined in every manager’s/leader’s job description?
Comment: We need to ensure that several clear statements of responsibility in loss control and safety exist in all job descriptions, with similar emphasis as that placed on cost effectiveness, quality assurance, productivity, and other areas of responsibilities. This should be applicable in all levels of management from supervisors to senior level Managers on site.
2. Do job documents refer to relevant regulatory requirements?
Comment: Reference to governmental regulations is required to strongly instill a personal responsibility to ensure a safe and healthy workplace. The intent of this question is not to require specific references to subsections of codes or regulations, but do require organizations to identify the major aspects of regulations which apply at the site and to make reference to them in job descriptions.
3. Is each individual provided with a copy or their job description?
4. Is the Safety performance of management personnel evaluated at least annually?
Comment: A review of the current appraisal procedures and/or appraisal forms should show evidence that the safety performance is being separately evaluated or included as an important consideration in the appraisal process.
5. Are the results of individuals’ safety performance evaluations documented?
6. Have employees been informed of their safety responsibilities with respect to:
Review of their work areas for potential losses and loss control/prevention opportunities? Report hazards to their immediate Supervisor?
Comment: To meet the intent of this question, there must be evidence that employees are made aware of their safety and health responsibilities, including such things as reviewing their work areas/tools/equipment for potential losses, reporting hazards, reporting accidents, following all applicable Safety and Health policies and regulations.
